All glaucoma surgeries are designed to lower eye pressure and stabilize or slow down the disease to reduce your risk of further vision loss. Traditional incisional (cutting) procedures are trabeculectomy and tube shunt surgery, which both serve to create an alternative drainage route that bypasses the eyes natural internal drain. They carry longer recover times and higher complication rates and are, generally speaking, reserved for more advanced stages of glaucoma. More recently, Minimally Invasive Glaucoma Surgeries (MIGS) have been developed and can often achieve pressure lowering with a better safety and recovery profile. MIGS target the internal drain of the eye and are often combined with cataract surgery. Examples of MIGS include iStent, goniotomy, and canaloplasty. Which particular surgery is best for you depends on many inter-related factors, including the level of your eye pressure, stage of disease, your age, and the anatomy of your eye.
